Dr. Laura Nicholas, Dr. Alyssa Trombley & Dr. Katie Schiavon are pleased to offer rapid-access, ultrasound-guided IUD management at Luna Health Clinic.

The Luna doctors completed their IUD training and practical internship through the Society of Obstetricians and Gynaecologists of Canada (SOGC). The doctors use an ultrasound-guided technique for IUD insertions, replacements and removals. If you would like have an IUD inserted, replaced or removed, please schedule an IUD Consultation as your first step. If you did not have your IUD inserted at Luna, but you would like your IUD checked for placement, you are welcome to book an IUD Consultation followed by an IUD Placement Check.

Procedure

An IUD is best inserted between Day 3-Day 15 of your menstrual cycle. We recommend you come in for insertion near the end of your period, or as soon as your period stops. Your cervix is naturally more open during this time, which makes for an easier insertion. At Luna Health Clinic, an endovaginal ultrasound is done prior to insertion to locate and visualize the shape of your uterus. This also ensures that you don’t have any structural abnormality which would exclude you as a candidate for an IUD. After that, a speculum is inserted, much like the beginning of a Pap test. Some freezing is applied to your cervix and then the depth of your uterus is measured. You will likely experience a little pinching and cramping at this time and during the insertion to follow. It generally takes less than 15 minutes from beginning to end. It is advisable that you take 650 mg of Acetaminophen (2 x regular strength Tylenol) or 400mg of Ibuprofen (2 x regular strength Advil) 1 hour prior to the appointment to minimize your discomfort. After the IUD is inserted, another ultrasound is preformed to ensure the IUD is in correct placement within your uterus. You will be asked to remain in the clinic for 15-20 minutes post insertion.

Side Effects

The initial side effects are cramping and spotting. The spotting usually lasts 3 to 14 days. Cramping is usually strongest the day of insertion, and tapers off within 24-48 hours. Infection is a potential side effect, and it more likely in those who have had multiple sexual partners in the last year, or who have a history of STIs, and STI test will be conducted in your appointment, and you will be contacted for treatment. if an STI infection is present. Infection will present with fever, abdominal pain and foul smelling discharge within 2 weeks after insertion. Perforation of the uterus happens in 1/1600 people. This is most common in postpartum women. This would be detected on your post procedure ultrasound, and a referral to your local hospital would be necessary to remove the IUD. The uterus would heal itself within a few weeks, with no impairment to future fertility.

- Copper IUDs
Can cause 30-50% heavier periods, 5% of women will expel the IUD within the first year. Copper IUD’s traditionally have a nickel core, which can cause local irritation in those women with nickel sensitivity. In 2019, a copper IUD with a silver core was approved by Health Canada. It is hypothesized that the silver core IUD may cause less inflammation and bleeding in those nickel sensitive women. Luna Health Clinic offers the silver core copper IUD preferentially, but you can make a request for the previous "standard" nickel core copper IUD instead. Please give us 1 week notice for "nickel core" requests so that we can be sure they are in stock in the clinic before your visit.

- Kyleena IUDs
Little to no hormonal side effects, however, ovarian cysts, depression, sexual changes, acne, and weight gain are possible. Most women maintain ovulation, with very light periods.

- Mirena IUDs
5-10% of women will have some hormonal side effects, including the possibility of ovarian cysts, depression, sexual changes, acne, weight gain, and breast tenderness. Most women will stop ovulating and lose their menstrual cycle (no periods). Aside from birth control, the Mirena IUD is also approved for the management of Menorrhagia (heavy menstrual bleeding) and Endometriosis.

Cost

The BC Government now covers the cost of IUD devices through MSP.
*If you do not have MSP, often times the Copper IUD, Kyleena and Mirena IUDs are covered or partially covered by extended health benefit drug plans. Please check with your insurance provider or pharmacist, as we cannot determine your coverage for these devices at Luna.
Copper IUD’s – $75.00. These can be purchased at the clinic at the time of your visit. Prescriptions can also be filled at the pharmacy and delivered to our clinic for your visit.
Kyleena and Mirena – approx $400.00. These are prescriptions that can be filled at the pharmacy and delivered to our clinic for your visit.

IUD Consultation (10 min, pre-assessment, virtual visit) – $75.00
IUD Insertion (30 min) – $200.00
IUD Removal and Insertion/Replacement (45 min) – $210.00
IUD Removal (30 min) – $95.00
IUD Complex Removal (45 min) – $120.00
IUD Placement Check (15 min) – $95.00
Pap test - $110.00
* Screening STI testing is available at visits for no additional charge. Pap testing can be done at the same time as the IUD procedures but there is an additional charge of $110

IUD Types

There are several Health Canada Approved IUDs. The following chart lists those approved IUDs that are offered and inserted at Luna Health Clinic. We are happy to insert other Health Canada approved IUDs upon request.
Liberte UT 380 Short (silver core) – 5yrs
Liberte UT 380 Standard (silver core) – 5yrs
Liberte UT 380 Short – 5yrs
Liberte UT 380 Standard – 10yrs
Liberte TT 380 Short – 5yrs
Liberte TT 380 Standard – 10 yrs
Kyleena (19.5mg levonorgestrel) – 5yrs
Mirena (50mg levonorgestrel) – 5yrs

- Copper IUD (Liberte UT and TT)
Copper IUDs are less expensive and have no hormones. They are very effective with about 1 in 100 people getting pregnant in a year. They usually make periods heavier, longer and sometimes more painful. There are no systemic side effects from a copper IUD, such as weight gain, mood changes, nausea, sore breasts, etc. Copper IUDs can have nickel cores or silver cores. It is estimated that 10-20% of women may have sensitivities to nickel, and is suggested a silver core copper IUD may not cause such dramatic increases in bleeding and cramping in those women.

- Kyleena 19.5mg
Low dose levonorgestrel (progesterone releasing). Most women will continue to have regular periods, but they are very light to perhaps just spotting. In studies, most people who use this dosage continue to ovulate, and have normal ovarian function. There are few systemic side effects, as the hormone dose is low and mostly contained to uterus.

- Mirena 50mg
Higher dose levonorgestrel (pregesterone releasing). About 10% of people will get the IUD removed for hormone side effects such as weight gain, mood changes or acne. Most people get some spotting in the first month or two after their Mirena has been inserted, but then period usually disappears for remainder of the 5 years. Often will inhibit ovulation completely.

We offer Point of Care (bedside) ultrasound imaging. This form of ultrasound is useful for monitoring parameters associated with early pregnancy, fertility, and gynecology.

This imaging is particularly helpful in early pregnancy for dating and tracking baby's growth, and offering reassurance to parents who have experienced miscarriage and pregnancy loss in the past. It is also useful for ruling in or out ectopic pregnancies.

This ultrasound imaging can be used for the purposes of fertility to do Antral Follicle Counts, and track the thickness of the endometrial lining.

This imaging is useful for monitoring ovarian cysts, PCOS, and fibroids. The imaging is not detailed enough to comment on endometriosis.

This type of ultrasound is beneficial for checking the placement of IUDs. To schedule an IUD placement check or to have an IUD inserted, removed or replaced, please go to our Discipline called "IUD Clinic" for all our IUD-related offerings.

If you have more questions, please email us at info@lunahealthclinic.com or give us a call at P: 604-229-7029.

If you are an active patient, you are welcome to book your ultrasound visits online. If you can a new patient you have to book an Ultrasound Consultation before you have access to booking an ultrasound visit.

Traditional Chinese Medicine (TCM) is ancient system of natural system of medicine. Acupuncture is used to circulate Qi (pronounced chi) and blood, and restore balance between organ systems. Acupuncture has been practiced for thousands of years, but more recently it has be studied in control trials which have revealed how acupuncture actually works! The essence of acupuncture is that the application of micro-injury increases local blood flow, facilitates healing and analgesia. Acupuncture involves the use of tiny needles to physically stimulate specific points in the body that activate neuro-chemical-hormonal-immune cascades in the body to cause a balancing effect (a return towards homeostasis).
Acupuncture...
1. Activates the neuroendocrine system - cytokines released at the acupuncture points cause the activation of the parasympathetic nervous system and the neuroendocrine system which controls the release of neurotransmitters, neuropeptides and hormones, which calm the body and mind regulate the release of appropriate hormones
2. Increases local blood flow - the release of neuropeptides and nitric oxide results in vasodilation, increases local circulation, and draws endorphins to the area
3. Increases cellular energy - mechanical stimulation increases intracellular ATP production to increase cellular energy, mitochondria function, decrease inflammation, decrease pain, and promote waste removal/detox

Acupuncture works particularly well for treating women's health concerns (fertility, pregnancy, menstruation), hormonal imbalances, stress, insomnia, mental health concerns, inflammation, pain, poor digestion, skin, acne and anti aging. All of our Naturopathic Doctors are trained in TCM and are board certified by the CNPBC to perform Acupuncture.

// Medical Microneedling (SkinPen Precision)

Microneedling is a safe, effective, and minimally invasive collagen induction therapy that can be used to treat a wide range of skin concerns such as fine lines, wrinkles, enlarged pores, acne scars, hyperpigmentation/melasma, uneven skin texture, and more. This action is achieved by the SkinPen device through the use of sterilized micro needles to create tiny channels within the skin that initiates a 3-step process to stimulate collagen and elastin production.

At Luna Health Clinic we onboard only most effective and safe treatments for our patients. For this reason, we have selected the SkinPen Precision Device. Unlike many other microneedling devices on the market, SkinPen is a MEDICAL GRADE microneedling device that has been FDA and Health Canada Approved.

We recommend a minimum of 3 sessions spaces 4-6 weeks apart. We have some patients who elect to come monthly all year round in order to treat and prevent signs of ageing, acne, and hyperpigmentation. Microneedling sessions pair nicely with LED Light Therapy. We recommend stacking this series back to back for enhanced results (book your Microneedling followed by a LED light therapy session and get the LED therapy for $60 instead of $100).

// Neuromodulator

Aesthetic neuromodulators are a type of injectable cosmetic treatment used to reduce the appearance of fine lines and wrinkles in the face. These treatments work by temporarily relaxing the muscles responsible for causing wrinkles and creases, resulting in smoother, younger-looking skin.
Aesthetic neuromodulators products contain a purified form of botulinum toxin, which is a neurotoxin produced by the bacterium Clostridium botulinum. When injected into the facial muscles responsible for causing wrinkles, the botulinum toxin blocks the signals sent from the nerves to the muscles, causing them to relax.
Aesthetic neuromodulators are typically used to treat dynamic wrinkles, which are caused by repeated facial expressions such as frowning, squinting, and smiling. They can be used to target wrinkles in the forehead, between the eyebrows, and around the eyes (crow’s feet), as well as to soften the appearance of a gummy smile and to smooth out neck bands.
The effects of aesthetic neuromodulators typically last for several months, after which the treatment can be repeated to maintain the results. The treatments are generally well-tolerated, but some patients may experience side effects such as bruising, swelling, and temporary muscle weakness. It is important to have these treatments performed by a qualified and experienced medical professional.

// Dermal Fillers

Aesthetic dermal fillers are a type of injectable cosmetic treatment used to add volume and contour to the face, as well as to smooth out wrinkles and fine lines. These treatments work by injecting a gel-like substance under the skin, which fills in hollow areas and plumps up the skin.
Aesthetic dermal fillers contain hyaluronic acid. Hyaluronic acid is a naturally occurring substance in the body that helps to hydrate and plump up the skin. When injected into the skin, hyaluronic acid dermal fillers can help to add volume and reduce the appearance of wrinkles and fine lines.
Aesthetic dermal fillers can be used to treat a variety of areas on the face, including the cheeks, lips, chin, and around the mouth. They can be used to add volume and definition to the lips, to smooth out nasolabial folds (the lines that run from the nose to the corners of the mouth), and to contour the cheeks and jawline.
The effects of aesthetic dermal fillers typically last for several months to a year, depending on the product used and the area treated. The treatments are generally well-tolerated, but some patients may experience side effects such as bruising, swelling, and temporary redness or tenderness at the injection site.
It is important to have aesthetic dermal fillers performed by a qualified and experienced medical professional who can tailor the treatment to your individual needs and goals.

// LED Light Therapy (Celluma iPro)

FDA-Approved light therapy for the purposes of: 1. Acne, 2. Wrinkles and aging, 3. Fertility - egg and sperm quality, 4. Pain and Inflammation, 5. Hair Restoration

Low-level-light therapy is the application of light energy to the body for therapeutic benefits. It promotes a natural photobiochemical reaction similar to the process of plant photosynthesis. The energy delivered by the LEDs has been shown to enhance cellular metabolism, accelerate the repair and replenishment of damaged skin cells, as well as stimulate the production of collagen — the foundation of healthy, smooth skin. Research has shown that LED light therapy may help enhance collagen production, accelerate tissue repair, kill acne bacteria, increase circulation, decrease inflammation, improve skin tone, smooth skin texture, improve skin firmness and resilience, increase lymphatic system activity, restore skin’s natural cellular activity, and reduce the appearance of fine lines, wrinkles, and superficial hyperpigmentation. LED light therapy has also been found to ease muscle spasms, decrease joint pain and stiffness, and reduce pain associated with arthritis.
Near-infrared light therapy has also sown great success in improving egg and sperm quality, thus increasing pregnancy success rates! It works by stimulating ATP production in the mitochondria which assisting in egg development. Light therapy increases pelvic blood flow and decreases inflammation which helps with implantation of the embryo. Research indicates the cell absorb particles of light (photons) and transform their energy into adenosine triphosphate (ATP), the form of energy that cells utilize. The resulting increase in ATP is then used to power metabolic processes; synthesizing DNA, RNA, proteins, enzymes and other products needed to repair or regenerate cell components; fostering mitosis or cell proliferation; and restoring homeostasis. Simply put, the LED light source provides compromised cells with added energy so the cellular performance is enhanced and repaired.
LED treatment is for all skin types, and is non-ablative, non-invasive, painless, and requires absolutely no downtime. Patients or clients can return to their normal activity immediately after the treatment. LED devices are considered extremely safe. Celluma LED light therapy is not approved for persons under 12 years of age. It should not be used over the abdomen of a pregnant person.
We kindly ask that you come to your LED session without makeup, lotion, or sunscreen over the area being treated. Many makeup formulas and cream contain minerals that may deflect light. Clean, freshly washed skin is recommended for best results.

Registered Clinical Counsellors are specialized mental health professionals with the ability to assess and treat mental health conditions and provide individual, couples or group therapy.

At Luna Health Clinic, it is our mission to offer compassionate and comprehensive healthcare for all life stages. We want to provide you with the practical tools and support system you need to navigate life's challenges. Our Registered Clinical Counsellor (RCC) is uniquely positioned to support you with your fertility, pregnancy, and parenthood journey.

Leeya Schachter completed her Bachelor of Arts degree at Simon Fraser University, where she majored in Psychology. She then went on to complete a Masters of Counselling degree from City University of Seattle. She holds a Registered Clinical Counsellor designation with the British Columbia Association of Clinical Counsellors.

Leeya works with women and their partners who are struggling to conceive, experiencing pregnancy loss, or undergoing fertility treatment and those who are suffering with pre and postpartum mental health challenges. She also supports people faced with childlessness after infertility.

Leeya understands that these experiences can feel incredibly isolating and unfortunately, still carry some stigma that can further exacerbate the isolation. Growing up, most of us were taught how common an unwanted pregnancy is, which led many of us to assume that getting pregnant (and staying pregnant) would be a simple straight-forward process. For 1 out of 6 couples in Canada, this is not the case. As an individual who has personally dealt with infertility, Leeya has experienced the anxiety, sadness, helplessness, grief, envy, shame, perfectionism, and anger that tracking cycles, managing the two week wait, and seeing others’ pregnancy announcements can trigger. She uses these experiences and her professional training to support her clients through deep empathy and understanding, helping them to process the complicated and conflicting feelings that come with these domains, as well as to develop coping strategies to manage this world of unknowns.

Leeya has a specialized skillset and counselling experience in perinatal mental health. She can help you with navigating:
- Fertility & Trying to Conceive (TTC)
- Infertility
- IUI & IVF & Assisted Reproductive Therapies
- Miscarriage and Recurrent Pregnancy Loss (RPL)
- Perinatal Mental Health
- Complex emotions surrounding Pregnancy & Birth

Within this session and future sessions, we will explore fertility related topics such as managing the two week wait, the feeling that your body is failing you, jealousy and envy towards others’ pregnancy announcements and how to cope with this, how to balance infertility with the rest of your life and identity, and many other topics. We welcome your feedback and requests to cover specific topics.

The benefits of support groups are twofold; participants have the opportunity to be supported and learn tools from the group facilitator and also to validate, empathize, and share experiences with one another. During the session, participants are encouraged to respond to and share ideas with one another and continue conversations with other participants following the session. By participating in this group, you will have the opportunity to create connections and build relationships with others who understand what it’s like to struggle to become pregnant or stay pregnant. Previous support group clients have shared that the connections and friendships they made in a support session have been maintained far past the conclusion of the group! If you choose to engage, there will be an opportunity to join a WhatsApp group in order to stay connected with the other participants in the support group between sessions.

Date & Time:
- Wednesday evenings 6pm-7:30pm
- Hosted virtually, on Zoom (join us from home, work, or wherever you are comfortable)
- Monthly sessions. Our first group will run for 6 months. You may choose to come to all sessions or just several sessions.

Cost & Insurance Coverage:
- The sessions cost $50/person/session
- The sessions are hosted by a Registered Clinic Counsellor and are eligible for submission to your extended medical insurance provider; these expenses are also eligible for tax reimbursement as a medical expense
